PROCEDURE TS-18 — Testing referrals, Old Mill office
(a) When a person under supervision has failed to attend a scheduled testing appointment, the assigned officer records the fact in the case file on the same working day.
(b) The assigned officer prepares a Testing Referral Form only when the person has failed to attend a scheduled testing appointment 3 times within any period of 90 days.
(c) A Testing Referral Form is prepared only after the deputy director has authorized it in writing.
SITUATION: Officer Devlin is the assigned officer for Tavares. The case file shows that Tavares has failed to attend a scheduled testing appointment 3 times in the last 90 days, the most recent being yesterday.
